When students need to present a doctor’s note, excusing their participation in physical education class, they are to:

• Bring it to the school nurse
• The nurse will keep it on file
• The nurse will give the student a medical form to present to the teacher
• The student should take this form to the teacher

Mr. Loftus will not accept notes from doctors, parents, friends or relatives. Any such instructions should be taken directly to the school nurse.
